SSM Health is looking for an experienced and qualified individual to join our diabetes management care team supporting five endocrinologists. The ideal candidate will be a Certified Diabetes Educator (CDE) through NCBDE and have previous experience with:
Managing patients with Type I and Type II Diabetes
Continuous Glucose monitoring systems
Insulin pumps
Managing a clinical patient care schedule
Working in Epic
Working with medication protocols under SSM Health physician guidance
The individual in this position will have their own patient schedule and be required to work in Janesville 1-2 days/week and Madison 3- days/week.
40 hours/week
